General Information

Applying for a U.S. Passport

Applications by U.S. citizen minors under 14 years require:

- Child must be present.
- Child's official, original birth certificate.
- Previous U.S. passport.
- One photo (see photo requirements).
- Both parents must be present with identification, preferably in English. Names of parents on the identification should match their names as shown on the child's birth certificate.
- If one parent cannot be present, that parent must complete and notarize form DS 3053 giving permission to the other parent to renew the child's passport.
- In case of death of one parent, the custodial parent should submit a Death Certificate.
- In case of divorce, both parents' signature or DS-3053 must still be presented unless one parent has sole custody. A divorce decree with custody authority granted by an appropriate court is required as proof of sole custody, with translation into English by a sworn translator.
- Photos of the progressive aging and physical changes of the child if the photo shown on the old passport does not reflect current physical appearance.

- Present Form DS - 11.
- Fee of US $82.00 or the equivalent in Lebanese currency.

Applications by U.S. citizens over the age of 14 require:
- Applicant must be present unless over the age of 26, with a previous 10-year passport which is valid, or expired within 5 years.
- Previous Passport.
- Photos of the progressive aging and physical changes if the photo shown on the old passport does not reflect current physical appearance.
- One photo (see photo requirements).
- Fees: Age 14 to 16 - US $82.00.(Form DS - 11); first passport after age 16, US $97.00.(Form DS - 11); subsequent renewals, US $67.00.(Form DSP - 82); the equivalent in Lebanese currency is acceptable for any of these fees.

- Passport renewals for U.S. citizens over the age of 26, who have had a 10-year passport which is valid, or expired within 5 years, may be mailed to the embassy using (Libanpost).

Reporting and Replacing a lost or Stolen Passport

The following must be submitted by the applicant in person:
- Police report, translated into English by a sworn translator.
- One photo (see photo requirements).
- Proof of US citizenship, if available, such as a U.S. birth certificate, Report of Birth Abroad, or Naturalization Certificate.
- Evidence to establish identity, such as a Driver's License, ID card, Lebanese or other passport.
- Present Form DSP - 64 and Form DS - 11.
- Fee of US $97.00, or the equivalent in Lebanese currency.

Amending a U.S. Passport

To change or correct a name or a date or place of birth in a passport, you must present a proof of the change, such as a Marriage certificate, Divorce decree, proof of legal name change through a court, or a birth certificate that reflects the exact date and place of birth. There is no fee for this service. Form DSP-19 should be filled out.

Amending a U.S. Passport

Form DSP-19 should be filled out and visa pages will be added on the spot.

Libanpost

Passport renewals for U.S. citiens over the age of 26, who have already had a 10-year passport which is valid, or expired within 5 years, may be mailed in to the embassy using Libanpost.

Drop off the previous passport, photo, application form (DSP-82) and certified check made out to the U.S. Embassy drawn on a U.S. bank for $67 at one of these branches of Libanpost.

Libanpost's fee is 8,800L.L. The new passport will be delivered to your home address in approximately two weeks.
